Register your team and show your team spirit by choosing a team color for your J4K Tournament T-Shirts with this registration option. PLUS…you save over $16.50 per player for a 12 person team when you bundle registration with your tournament tee! Simply nominate a Team Captain to register your team of 10 to 12 players. Your Team Captain can select one of the available color choices listed on the Team Registration form that pops up when you click the “Register Team” button below. We will contact each Team Captain 3-weeks prior to the event for a finalized team roster and t-shirt order.
Color Choices are Lime Shock, Silver, Neon Yellow, Atomic Blue and Vegas Gold.
Teams must have a minimum of 3 (three) Women Players

DISCLAIMER CLAUSE: (Justice4Katie) Its members, directors, officers, agents, representatives and/or volunteers (hereafter “J4K”) are not responsible for any injury, loss or damage of any kind sustained by any person while participating in the 2nd Annual Kicks for Katie Adult Kickball Tournament (the “Kickball Tournament”), including injury, loss or damage which might be caused by the negligence of the “Kickball Tournament”.
PHOTOGRAPHY RELEASE: I, as the subject of photographs/video taken on this date, hereby consent and authorize the use and production of the photographs/videos by J4K. These same photographs/video may be used for any purpose including, but not limited to, promotion, advertising and public relations without further compensation or consent.
ASSUMPTION OF RISKS: In consideration of my participation in the Kickball Tournament, I acknowledge that I am aware of the possible risks, dangers and hazards associated with my participation in this event, (including the possible risk of severe or fatal injury to myself or others).
